ASQ-3

The Ages & Stages Questionnaire (ASQ-3) is a quick, engaging tool that helps you learn more about your child’s development—from motor skills to communication and problem-solving. It’s designed for children from 1 month to 5½ years old and helps families spot strengths and identify areas where more support may be helpful. You’ll get clear, caring feedback and suggestions for next steps if needed.

Who Can Do ASQ‑3?
  • Designed for children aged 1 month to 5½ years, with 21 age-specific questionnaires

  • Caregivers complete the questionnaire (online or on paper), ideally during your child's routine check-ins or program enrollment. Results are reviewed with you, and referrals are made if any developmental areas need follow-up

  • Must be a resident of Oakland County

ASQ:SE-2

The Ages & Stages Questionnaire: Social-Emotional, Second Edition (ASQ:SE-2) focuses on how your child is doing socially and emotionally—things like managing feelings, getting along with others, and handling changes in routine. It’s a great way to check in on your child’s emotional well-being and learn how to support healthy development from the start.

Who Can Do ASQ:SE‑2?
  • For children aged 1 to 72 months (up to 6 years)

  • It's a social-emotional screener, also parent-led, done soon after enrollment or during check-ins. It flags any behavioral concerns and helps guide next steps

  • Must be a resident of Oakland County
